Job Summary Homeland Talent Solutions is partnering with a leading healthcare technology organization to hire Licensed Practical Nurses (LPN/LVNs) to support its growing Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs. This is a fully remote clinical role designed for an LPN who is comfortable assessing patient concerns, reviewing vital signs and remote monitoring data, conducting patient outreach, and determining when concerns require additional clinical escalation.
The LPN will serve as an important clinical escalation resource for patients with chronic health conditions. When RPM technology identifies an abnormal or out-of-range patient reading, the LPN may review the available clinical information, contact the patient to further assess the situation, determine the appropriate next step within LPN scope of practice, and escalate more complex or urgent concerns to the RN Escalation team.
This is an excellent opportunity for an LPN with strong triage, telehealth, clinical escalation, remote patient monitoring, chronic care, or similar patient-assessment experience who enjoys using clinical judgment in a remote environment.
Responsibilities
Monitor and respond to Remote Patient Monitoring alerts and out-of-range patient readings
Review available patient history, vital signs, RPM readings, and other relevant clinical information to determine urgency and appropriate next steps
Conduct primarily outbound patient outreach related to abnormal readings, clinical concerns, and escalations
Ask appropriate clinical questions and assess patient concerns within LPN scope of practice
Resolve patient concerns that can appropriately be handled at the LPN level
Promptly escalate complex, urgent, or higher-acuity situations requiring RN or provider-level assessment
Serve as a clinical escalation resource for care management, inbound, outbound, and enrollment teams when patient concerns exceed the initial team member's scope
Communicate with provider offices and other members of the care team as appropriate to coordinate patient care
Support patients participating in both RPM and Chronic Care Management (CCM) programs
Accurately document patient assessments, outreach, escalations, communications, and outcomes
Follow established clinical escalation protocols while exercising appropriate independent clinical judgment
Maintain established productivity, quality, documentation, and patient-care standards
Requirements
Current, active LPN/LVN license with Compact/Multi-State privileges required
LPN/LVN degree from an accredited nursing program
Strong clinical assessment, critical-thinking, and decision-making skills
Ability to recognize potentially urgent patient concerns and determine the appropriate escalation pathway
Comfortable conducting patient outreach and discussing symptoms, vital signs, chronic conditions, and other clinical concerns
Ability to work independently and exercise sound judgment within LPN scope of practice
Strong written and verbal communication skills
Ability to remain composed and organized while managing multiple patient concerns and changing priorities
Strong documentation skills and ability to follow established clinical protocols
Comfortable working in a technology-enabled, fully remote healthcare environment
Additional Information There are currently four openings, with one LPN being hired for each schedule: Schedule 1: Monday–Friday | 3:30 PM–12:30 AM EST Schedule 2: Monday & Tuesday | 8:00 AM–7:00 PM EST Wednesday & Sunday | 7:00 AM–6:00 PM EST Schedule 3: Wednesday, Thursday & Friday | 8:00 AM–7:00 PM EST Saturday | 7:00 AM–6:00 PM EST Schedule 4: Monday & Tuesday | 10:00 AM–8:00 PM EST Thursday & Friday | 1:00 PM–11:00 PM EST Candidates must be able to consistently work one of the designated schedules above.
