Job Summary Homeland Talent Solutions has partnered with a leading national healthcare organization to hire Registered Nurses (RNs) – Clinical Escalation to join a growing remote clinical care team supporting Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) patient populations.
The RN Clinical Escalation Nurse serves as a senior clinical resource responsible for assessing complex or urgent patient concerns, evaluating RPM alerts, and determining the appropriate clinical intervention. This position is ideal for an experienced RN who is comfortable applying independent clinical judgment and making autonomous decisions in a fast-paced remote environment.
Responsibilities
Serve as an RN-level escalation resource for CCM and RPM patient populations
Assess complex or urgent patient concerns escalated by LPNs and other care management teams
Evaluate out-of-range RPM readings and clinical alerts
Apply independent clinical judgment to determine urgency and appropriate intervention
Conduct direct patient outreach regarding urgent or complex clinical concerns
Determine when situations require provider notification, emergency services referral, or immediate intervention
Communicate directly with ordering providers, specialists, and care teams to coordinate resolution of escalated concerns
Provide clinical guidance to LPN team members on complex cases within RN scope of practice
Accurately document assessments, communications, interventions, and outcomes in accordance with clinical and regulatory standards
Identify trends in escalations or RPM alerts that may warrant care plan revisions
Maintain composure and sound clinical decision-making during high-acuity or time-sensitive situations
Collaborate with clinical and care management teams to support positive patient outcomes
Requirements
ADN or BSN from an accredited nursing program
Active New York RN license
Active NLC multistate RN license
Prior triage, telehealth, Emergency Department, urgent care, or similar clinical experience preferred
Strong independent clinical judgment
Comfortable making autonomous clinical decisions in a remote environment
Excellent patient, provider, and interdisciplinary communication skills
Ability to remain composed and effective during urgent or high-acuity situations
Comfortable working within a technology-enabled remote clinical environment
Flexibility to work an assigned schedule, which may include second-shift or weekend coverage
Additional Information Candidates must meet both licensing requirements below:
Hold an active New York RN license
Hold an active Nurse Licensure Compact (NLC) multistate
RN license Candidates may currently reside in New York or another eligible state
Both the New York license and active multistate compact license are required
Compensation/Benefits
Base Salary: $65,000 annually
Bilingual Differential: Additional $1.00/hour equivalent / approximately $2,080 annually
