Job Summary Homeland Talent Solutions has partnered with the nation’s largest provider of Chronic Care Management (CCM) services to hire a New York-licensed LPN Personal Care Coordinator to join the team. Our client helps patients with multiple chronic conditions stay connected to their providers and improve long-term health outcomes, offering a fully remote role with a consistent schedule and meaningful patient interaction.
Candidates must hold an active New York LPN license. Candidates who also hold a multistate Nurse Licensure Compact (NLC) license in addition to their New York license are strongly preferred.
The LPN Personal Care Coordinator supports healthcare providers by delivering Chronic Care Management services through proactive outreach, education, and care coordination. This role helps ensure patients stay engaged in their care between office visits.
In this role, the LPN Personal Care Coordinator partners with patients and care teams to identify needs, remove barriers, and connect patients to appropriate resources—driving improved outcomes and continuity of care.
Responsibilities
Conduct monthly outbound care management calls to assigned patients
Identify patient healthcare needs and coordinate appropriate resources
Develop and support patient-specific goals and care interventions
Provide health education and lifestyle guidance
Maintain accurate and up-to-date patient records, including medical history, medications, immunizations, allergies, surgical history, and family history
Ensure all documentation meets compliance and regulatory standards
Update care plans based on patient needs and identified barriers to care
Partner with care teams to support and improve patient outcomes
Escalate clinical concerns to the triage nurse team as needed
Contribute to a collaborative, team-oriented environment
Requirements
Active, unrestricted New York LPN license
LPN diploma from an accredited nursing program
Strong knowledge of and adherence to clinical protocols and procedures
Excellent communication and patient engagement skills
Ability to multitask and adapt in a fast-paced environment
Team-oriented mindset with a collaborative approach
Experience in care coordination or chronic care management, preferred
Previous remote nursing or telehealth experience, preferred
Familiarity with EMR systems, preferred
Active multistate Nurse Licensure Compact (NLC) license in addition to New York licensure, strongly preferred
Additional Information This is a fully remote position with company-provided equipment. A professional home office environment is required. The role involves daily use of phone and computer systems and follows a Monday through Friday schedule during standard daytime hours.
